Better Health Pathways HUB - Referrals

When you refer someone to our Pathways HUB, we share your referral with one of the HUB's contracted care coordination agencies. The agency assigns a community health worker who will reach out to partner and guide the referred individual. 

To send a referral use one of the following options below. A Better Health Pathways HUB team member will follow up within 72 business hours of receipt of the referral.

This form requests minimum necessary information related to the referral and is submitted via the website.
