We take great pride in celebrating our member successes – especially those that achieve or exceed our goals for regional health improvement and the reduction of health disparities. Every year we acknowledge our clinical members as "Gold Star" Practices who have improved the most or achieved the highest outcomes related to the populations they serve.
We also celebrate our Pathways HUB care coordination agency partners and their community health workers related to their client successes and achievements in meeting health, social and economic needs.
We feature member successes in our newsletters, on social media, webinars, and Better Health's annual report to the community. We truly believe that by taking the time to acknowledge and celebrate successes, we reinforce the motivation of the collaborative members to go the distance for continuously raising the bar and accomplishing their shared goals.
