Better Health Partnership’s regional collective impact is only as strong as its’ members and partners’ commitment of their time, talent and treasure to our mission of achieving equitable population and community health.

Members, together with Better Health’s team set direction and execute strategic priorities and metrics in alignment with the needs of the community and populations served. We value the diversity of our members’ experiences and approaches to innovation and implementation of evidence-based solutions that lead us to positive, sustainable change.

BHP’s collaborative has grown from an original membership of just two health systems, to over 2,000 primary care provider members who report on nearly 600,000 patients they collectively serve across seventeen counties in Northeast Ohio.

Together, these providers have achieved significant improvements in blood pressure control and the reduction of racial and ethnic disparities in diabetes care, and avoided nearly 6,000 ambulatory care sensitive hospitalizations in Cuyahoga County.

With the addition of the Pathways HUB, our collaborative now includes many social service providers and care coordination agencies, as well as payers, public health, and community-based organizations.
