Better Health Pathways HUB Community Advisory Board

The Pathways HUB Community Advisory Board helps guide direction, strategies and quality improvement activities of the BetterHealth Pathways HUB (HUB) in alignment with goals for improving health, reducing disparities, and meeting social, educational, and economic needs of Cuyahoga County's most vulnerable residents. The Board consists of diverse organizations and persons who understand and can best represent the clients served by the HUB, including, but not limited to: Care Coordination Agencies, Community Health Workers, clients of the HUB, health systems, payers, city and local government, public health, social service providers, business, philanthropy, and other community organizations.
