Marie Velez, Better Health Pathways HUB Supervisor, was recently featured as a panelist on a Partnership to Align Social Care webinar, “Exploring Core Functions of Community Care Hubs: A Look at Leadership, Governance, and Business Development.” Joining Marie were representatives from California and Wisconsin including community care hub leaders, health plan partners, and Partnership to Align Social Care experts who shared:
- An overview of the community care hub model and background on the development of the community care hub concept and its value proposition.
- The functions and capacities of a community care hub, particularly around leadership, governance, and strategic business development.
- Perspectives from community-based organizations and health care leaders highlighting best practices where community care hubs have effectively aligned on leadership, governance, and strategic business development to address health-related social needs for multiple populations.
