Better Health Pathways HUB Achieves National Re-Certification

We proudly share the news of our Better Health Pathways HUB recently achieving a three-year, Level I Pathways Community HUB Institute (PCHI) re-certification!

PCHI, the nation’s only certifying body for community-based care coordination Pathways Community HUBs, provides the Level I designation to organizations that meet at least 90% of the national standards for quality community care coordination services and who demonstrate a commitment to pursuing excellence. Organizations seeking certification go through a rigorous peer review process to demonstrate their commitment to offering programs and services that are measurable, accountable and of the highest quality.
