Better Health Partnership’s Pathways HUB Expands Into Medicare Market with Medical Mutual of Ohio Pilot

We are thrilled to announce the launch of a new initiative that will pilot the provision of Better Health Partnership’s (BHP) Pathways HUB services to Medicare members, in partnership with BHP member organization Medical Mutual of Ohio. BHP’s Pathways HUB, which has served Medicaid clients in Cuyahoga County since 2020, is an evidence-based care coordination model that helps individuals obtain needed care and resources by connecting them with community health workers.

"We are thrilled to collaborate with the Better Health Partnership Pathways HUB team to launch this pilot for our valued Medical Mutual Medicare members," stated Betsy Williamson, Vice President of Cost of Care & Population Health at Medical Mutual. "We appreciate the Pathways Community HUB Institute for promoting the HUB model as an evidence-based method to identify and address unmet social needs. Our involvement in the design phase of this pilot has been instrumental in ensuring our focus on being a trusted navigator for our members. This opportunity enhances our ongoing partnership with BHP, allowing us to further positively impact the lives of our members in Cuyahoga County.”

While it is often assumed that unmet social needs exclusively impact individuals with Medicaid coverage or those who are uninsured, research demonstrates that people with various types of insurance, including Medicare, often experience similar challenges. “The HUB and its network of care coordination agencies have helped nearly 4,000 individuals with Medicaid coverage navigate complex health and social challenges since its inception in 2020,” said Dr. Robert Eick, President and CEO of Better Health Partnership. “We are excited to leverage this model to serve the Medicare population – a first step toward addressing unmet needs at scale for new populations. As the proportion of the 65+ population continues to grow, finding effective ways to address unmet medical and social needs will be essential.”
