In this section, we proudly share examples of regional achievements with improved health outcomes, reduced disparities in healthcare delivery, and avoided ambulatory care sensitive hospitalizations accomplished by Better Health’s collaborative partners which have been widely published and nationally recognized.
Additionally, we share the many ways that our collaborative partners have influenced policy changes for medicaid expansion and new models of health care delivery aligned with CMS and innovative payment.
