Funders, Payers and Supporters

We express our sincerest appreciation for the past and present funders, payers, and supporters of the Better Health Pathways HUB. This funding helps us provide the infrastructure necessary to operate a network of care coordination agencies that serve Cuyahoga County at-risk residents, provide them with reimbursement from Medicaid Managed Care to help sustain their organizations, and to capture data in a common software system used to evaluate outcomes and inform advocacy efforts for minority populations.
