Better Health Partnership offers social care coordination through its certified Pathways HUB, a network of care coordination agencies serving individuals and their families in Cuyahoga County to overcome health and social barriers. We value the trusted relationships we have with our care coordination agency (CCA) partners to meet a wide variety of health and social care needs in our community. CCAs in the HUB use the Pathways Community HUB Institute (PCHI) model as a standard way to guide their clients towards improved outcomes following health and social "pathways", and they assign a trusted community health workers (CHW) to serve clients as members of the HUB.
